Dementia Care Planning

I have recently been invited to join the Center for Memory and Aging, a neurology practice in Poway, CA. Their focus is evaluation and treatment of memory impairment and cognitive deficits. My role as Geriatric Care Manager will be to along with the team, develop Individualized Dementia Care Plans. The IDCP includes educational material, community resources and caregiver support all customed tailored to each family's needs. I am honored to be working with Dr Ho and his team.
